2024 Arizona Complete Health-Long Term Care Plan Provider Forums

Arizona Complete Health-Long Term Care (AzCH-LTC) is honored to be serving Arizona Long Term Care members effective October 1, 2024. We are excited to build upon a tradition of serving Arizonans’ statewide as an ACC, RBHA, DSNP, Marketplace Plan. Member continuity is our top priority as we implement our Arizona Long Term Care System (ALTCS) program.
We are looking forward to partnering with you to ensure a smooth transition for your members and to serve this highly vulnerable population with the goal of improving outcomes and assisting Arizonans’ to age in place.
AzCH-LTC and the Provider Engagement Team are hosting numerous Provider Forums throughout the state.We invite you to attend one of the upcoming provider forums where we will share information on Arizona Complete Health, member transition and address any questions you have.
