Integrated Care
Arizona Complete Health Integrated Care Teams are available to assist members with complex medical, behavioral or social needs that may benefit from increased coordination of services to optimize health and prevent disease. Our Care Management Care Coordination Department is staffed by individuals with extensive medical and behavioral health experience in areas such as obstetrics, oncology, behavioral health, housing, employment, substance use disorders, medical/surgical, HIV/AIDS, and physical rehabilitation for children and adults.
Early intervention is essential to maximize treatment options while minimizing potential complications associated with catastrophic illnesses or injury and exacerbation of chronic conditions. Care Coordinators and Care Managers are available from 8 a.m. to 5 p.m. seven days a week to assist with coordination of members’ healthcare needs.
The following conditions and/or diagnoses are examples of appropriate referrals to the Care Coordination Care Management Department:
- Adults and children with Serious or Complex Medical and/or Behavioral Needs
- Complex social challenges (social isolation, hunger, housing, domestic violence)
- Asthma
- Diabetes
- High Blood Pressure
- Heart Conditions
- COPD
- Substance Use Disorders
- Mental Health Disorders
The provider’s role in Arizona Complete Health's Care Management Care Coordination Program is extremely important.
