Arizona Complete Health and Care1st are introducing a new template for requesting Transcranial Magnetic Stimulation (TMS). Please complete and include it along with the prior authorization form and applicable medical records, when requesting TMS services for members. It will help us to more quickly and accurately complete review of your request.
