Transcranial Magnetic Stimulation (TMS) Unit Update

Dear Providers and Staff:

Effective immediately, Arizona Complete Health and Care1st Transcranial Magnetic Stimulation (TMS) providers can request 36 units of TMS to be reviewed by a medical director. Per standard of practice, please limit your request to no more than 36 units.

Complete and submit the new TMS Initial Request Form along with the prior authorization form and all pertinent clinical information with your request.
