Medical Records Submission to Arizona Complete Health (AzCH) & Care1st

AzCH & Care1st are committed to supporting our providers in reducing administrative burden and maximizing earning potential. It is important that you know your options for submitting records and/or allowing our team access to your Electronic Medical Record (EMR). Simply choose an option below!

Option 1: Grant the Quality HEDIS Team Remote Access (EMR Access)

• Allow us to abstract medical records without the administrative burden of record requests.
• Please email HEDIS_Operations@azcompletehealth.com or connect with your assigned Provider Quality Liaison to set up this option.

Option 2: Email Medical Records Securely to our Quality HEDIS Team

• Send a secured email: HEDIS_Operations@azcompletehealth.com.
• This option allows for confirmation of receipt from our AzCH/Care1st Quality HEDIS Team.

Option 3: Fax Medicare Records to our Quality HEDIS Team

• Fax # 1-855-872-1858

Option 4: Secure SFTP

• Email a request to   HEDIS_Operations@azcompletehealth.com and we will begin the process to set up a secure SFTP site to share records.
• If this method is used, we request an email HEDIS_Operations@azcompletehealth.com for each submission to let our Quality HEDIS Team know records were placed on the secure SFTP.

Option 5: Schedule Onsite

• Contact us to schedule an onsite to pick up medical records.
• Please email HEDIS_Operations@azcompletehealth.com or request a pick-up with your assigned Provider Quality Liaison.

Option 6: Secure Provider Portal

• Medical record submission is available via the secure AzCH or Care1st provider portal.
• This option does not allow for confirmation of receipt by our Quality HEDIS Team.
