Important notice: Laboratory referrals

Sonora Quest Laboratories is the statewide exclusive contracted outpatient lab for Arizona Complete Health (AzCH) (Medicaid, Medicare, and Marketplace) and Care1st members.  Sonora Quest is a full-service lab and can perform all lab tests, including complete drug screenings and maternity workups.

  • Members directed for lab testing should be sent only to Sonora Quest
  • Labs drawn in the office and sent out for processing should be sent only to Sonora Quest
  • If your practice has another laboratory on site, please work with that lab to forward blood draws and specimens for AzCH and Care1st members to Sonora Quest to avoid claim denials for services referred to non-contracted laboratories.

For a listing of Sonora Quest patient service locations, visit www.sonoraquest.com and select the “Locations & Appointments” tab at the top of the home page.  Web-based patient service center appointment scheduling is available and offers members the ability to schedule an appointment for a convenient day and time, resulting in reduced wait time upon arrival at a patient service center.  The web-based scheduling system is available 24 hours a day.  Although appointments are encouraged, walk in appointments are also available during scheduled hours of operation.
