Update to Ambetter Pharmacy Prior Authorization Request Fax Number

At Ambetter from Arizona Complete Health, we are always looking for ways to provide a better service for your patient, our member.

In this continued pursuit, we are working with our Pharmacy Benefit Manager (PBM) in streamlining the prior authorization (PA) process to ensure your patients, our members, have their drug authorization request reviewed timely and efficiently.

We have updated the Ambetter pharmacy PA request fax number on our pharmacy PA request forms, which can be used today. The new fax number is (800) 977-4170.
