Race, Ethnicity, Language, Sexual Orientation and Gender Identity Information
Arizona Complete Health is committed to keeping your race, ethnicity, and language (REL), and sexual orientation and gender identity (SOGI) information confidential. We use some of the following methods to protect your information:
- Maintaining paper documents in locked file cabinets
- Requiring that all electronic information remain on physically secure media
- Maintaining your electronic information in password-protected files
We may use or disclose your REL and SOGI information to perform our operations. These activities may include:
- Designing intervention programs
- Designing and directing outreach materials
- Informing health care practitioners and providers about your language needs
- Assessing health care disparities
We will never use your REL and SOGI information for underwriting, rate setting or benefit determinations or disclose your REL or SOGI information to unauthorized individuals.
Company’s Privacy Policy
This Company Privacy Policy discloses the information privacy practices for the website you are currently visiting. Throughout the policy, we refer to information that can identify you as a specific individual, such as your name, phone number, email address, Social Security number or credit card number, as “personal information.” Further, personal information includes any information involving your health or medical history. We do not sell, trade or give away your personal information to anyone.
We do not disclose your personal information to third parties, unless one of the following limited exceptions applies.
If you submit an application for an insurance product offered through an insurance agent or agency with whom we have associated, then we may disclose your personal information to that agent or agency to process your requested quote or application. Please be assured that these partners are only allowed to use your personal information to process your requested quote, application or enrollment form and are contractually obligated to maintain strict confidentiality.
Service Providers. We may disclose your personal information to other companies that help us process or service your insurance application or correspond with you. For example, we may provide your personal information to a service provider to verify the validity or credit limit of your credit card. Please be assured that the companies we hire to process or service your insurance application or correspond with you are not allowed to use your personal information for their own purposes and are contractually obligated to maintain strict confidentiality.
Legal Obligations. We may disclose or report your personal information when we believe, in good faith, that the disclosure is required or permitted under law, for example, to cooperate with regulators or law enforcement authorities or to resolve consumer disputes.
Outside of these exceptions, we will not share your personal information with third parties. We collect personal information in the normal course of business in order to process your insurance application and to serve you better.
Registering With Us. We begin collecting personal information from you when you register with us. You may register with us through our website by entering your email address and, if you are creating an account, choosing your password. Under our current process, you must create an account before starting an application for a health insurance or other product offered on our website.
CHILDREN UNDER THE AGE OF 13 YEARS. In accordance with the Children’s Online Privacy Protection Act (COPPA), we will not knowingly collect or accept personally identifiable information from a child under the age of 13 without a parent’s or guardian’s prior consent. We will also allow the parent or guardian of a user under the age of 13 to review, update, change, delete or refuse to permit further collection of any and all information collected by the website. A child’s personally identifiable information will be treated with the same care and respect as an adult’s information.
Applying for Health Insurance. If you apply for health insurance through our website, we may ask you to provide us with personal information and/or health information relating to you and any family member who will be included on your application. This information will be used to process your application. Additionally, we may ask you to provide us with credit card or bank information, which will be used to process your application or collect any fees associated with your application or insurance premiums upon approval of your application.
Providing you with a quote or processing your application. We may use your personal information to get in touch with you when necessary to process your application or to provide you with a quote. For example, emails may be sent to you throughout the application process to inform you of the status of your application and to seek additional information that is requested as part of the application.
Customer satisfaction, referrals and other products. We may contact you to survey your satisfaction of our service, refer our products and services to other people, and/or to inform you of additional products and services.
We gather anonymous information about you for our internal purposes, and we may share this anonymous information with third parties.
Anonymous information is any information that does not personally identify you, including aggregate demographic information such as the number of visitors to our website from a particular state. We use anonymous information primarily for marketing purposes and to improve the services we offer you.
We protect the confidentiality and security of your personal information.

Additional Information About Privacy and Security
“Cookies” and “Clear Gifs”
“Cookies” are small files that are stored by your web browser to help a particular system recognize you and the pages you visited in a website. Our website uses cookies to make your online experience more convenient. For example, we may use a cookie to store your account information between sessions and to maintain information about the quotes you have requested during your session. Additionally, we may use data from cookies for a variety of internal purposes, such as studying how users navigate our website. Most browsers automatically accept cookies, but if you prefer, you can set yours to refuse cookies. Even without a cookie, you can still use most of the features on our website, including obtaining quotes and applying for an insurance policy.
“Clear gifs” are tiny graphics with a unique identifier, similar in function to cookies, that are used to track the online movements of website users. The main difference between the two is that clear gifs are invisible on the page and are much smaller than cookies.
We use third party web analytics services to track and analyze anonymous usage and volume statistical information from visitors. These analytics help us administer our website, improve our website’s performance and to report website usage traffic. These web analytics services use cookies, clear gifs and other web monitoring technologies to help track visitor behavior on our behalf.
We also use reCAPTCHA, a service provided by Google, to help protect our websites from spam and various types of automated abuse. reCAPTCHA does this by collecting hardware and software information, such as device and application data and the results of integrity checks, and sending that data to Google to analyze whether an input is made by a human or an automated process. Browser Support and Internet Security
Any time you enter or provide personal information in our website, we encrypt it using Transport Layer Security (TLS) 1.2 or better technology. TLS protects information as it crosses the Internet. To support this technology you need to use a browser with strong encryption, such as Mozilla Firefox, Microsoft Edge v77+, Safari, and Google Chrome, and update your browser versions when prompted to ensure you have the latest security capabilities. In order to adhere to the latest standards and ensure the highest levels of security, we generally support only the most recent major release of a browser, plus one version previous for compatibility.
In the specific case of web browser support, the following four browsers are supported in accordance with the vendor support policy listed:

Microsoft Internet Explorer policy update:
Microsoft Internet Explorer 11- Microsoft has deprecated IE 11 in Windows 10 and recommends using Edge as the default browser.
Given that Microsoft has deprecated Internet Explorer 11, beginning in July 2020 Centene will stop supporting Internet Explorer as a valid browser client to access our sites. There are significant security and performance concerns with Internet Explorer 11 (see Microsoft FAQ), and Centene encourages customers to move to a modern browser such as Microsoft Edge, Google Chrome, Mozilla Firefox, or Apple Safari.
For mobile device operating systems, Centene provides support for the most recent browser delivered by the device operating system only.

In the case where feature parity is not consistent for the same browser version across platforms and devices, those features are only supported by Centene to the extent that the vendor has enabled.
Your Password and Other Security Issues
The password you enter when you begin an application on our website is your private entry key into your application. You should never share it with anyone. Your password must be between 6-20 digits and/or letters long. If you forget your password, click on “Forgot Your E-Mail Address or Your Password?” on the quoting page.
Security Risk of Using Non-Approved Automated Software Applications
For security reasons to guard the safety of your data, access to this website is limited to SSL-capable browsers such as Microsoft’s Internet Explorer 4.01 or higher or Netscape Navigator 4.06 or higher. Under no circumstance should you use any software, program, application or any other device to access or log-in to the Company website, or to automate the process of obtaining, downloading, transferring or transmitting any content to or from our computer systems, website or proprietary software.

1. Sources of Personal Data
We collect Personal Data about you from the following sources:
- Directly from you. We may collect Personal Data you provide to us directly, such as when you contact us through our Digital Properties, submit orders, complete questionnaires or surveys, or sign up for newsletters through our Digital Properties.

3. How We Use Personal Data

A. Types of cookies and tracking technologies we use
We and the third parties that we authorize may use the following tracking technologies:
- Cookies, which are a type of technology that install a small amount of information on a user's computer or other device when they visit a website. Some cookies exist only during a single session and some are persistent over multiple sessions over time.
- Pixels, web beacons, and tags, which are types of code or transparent graphics. In addition to the uses described below, these technologies provide analytical information about the user experience and help us customize our marketing activities. In contrast to cookies, which are stored on a user's computer hard drive, pixels, web beacons, and tags are embedded invisibly on web pages.
- Session replay tools, which record your interactions with our Digital Properties, such as how you move throughout our Digital Properties and engage with our webforms. In addition to the uses described below, this information helps us improve our Digital Properties and identify and fix technical issues visitors may be having with our Digital Properties.
- Embedded scripts and SDKs, which allow us to build and integrate custom apps and experiences on our Digital Properties

We and authorized third parties use these technologies for purposes including:

C. Information collected
These tracking technologies collect data about you and your device, such as IP address, cookie ID, device ID, Ad ID, your approximate location based on IP address, operating system, device type, browser used, browsing history, search history, and information about how you interact with our Digital Properties (such as pages on our Digital Properties that you have viewed).
D. Disclosures of your information
We may disclose information to third parties or allow third parties to directly collect information using these technologies on our Digital Properties, such as social media companies, advertising networks, companies that provide analytics including ad tracking and reporting, security providers, and others that help us operate our business and Digital Properties.
E. Your choices
Some of the third parties we work with participate with the Digital Advertising Alliance ("DAA") and Network Advertising Initiative ("NAI"). The DAA and NAI provide mechanisms for you to opt out of interest-based advertising performed by participating members at http://www.aboutads.info/choices/ and https://optout.networkadvertising.org/. Opting out of interest-based advertising will not opt you out of all advertising, but rather only interest-based advertising from us or our agents or representatives.
You can also refuse or delete cookies using your browser settings. If you refuse or delete cookies, some of our Digital Properties’ functionality may be impaired. Please refer to your browser’s Help instructions to learn more about how to manage cookies and the use of other tracking technologies. If you change computers, devices, or browsers; use multiple computers, devices, or browsers; or delete your cookies, you may need to repeat this process for each computer, device, or browser.
Some browsers have incorporated Do Not Track (“DNT”) preferences. At this time, we do not honor Do Not Track signals.
6. Other Choices
In addition to the choices related to cookies described above, you may exercise the following choices about how we use your Personal Data:
- Marketing Email Opt-out: We may send you emails about our products or services or other marketing emails from time to time. You can unsubscribe from these emails by clicking the unsubscribe link in the email. We may continue to send you important emails about our relationship with you, such as updates about your purchases.
- Mobile Apps: Our Digital Properties may include mobile applications. If you download a mobile application from us, you may be able to adjust the personal information we collect through the application, including as explained below.
- Location Information: Our mobile applications may collect location information. You may opt-out of sharing location information by adjusting the location settings on your mobile device. We may otherwise still collect information that estimates your location based on your IP address.
- Push Notifications: Our mobile applications may offer automatic or “push” notifications. You may opt-out of push notifications by adjusting your device settings.
7. Data Security and Data Retention
Although we maintain reasonable security safeguards, no security measures or communications over the Internet can be 100% secure, and we cannot guarantee the security of your information.
Any transmission of personal information is at your own risk. We are not responsible for circumvention of any privacy settings or security measures contained in the Digital Properties.
The safety and security of your information also depends on you. Where we have given you (or where you have chosen) a password for access to certain parts of our Digital Properties, you are responsible for keeping this password confidential.
Your Personal Data will be retained as long as necessary to fulfill the purposes we have outlined above unless we are required to do otherwise by applicable law. This includes retaining your Personal Data to provide you or your company with the services you have requested and interact with you; maintain our relationship with you; improve our business over time; ensure the ongoing legality, safety and security of our services and relationships; or otherwise in accordance with our internal retention procedures. Once you have terminated your relationship with us, we may retain your Personal Data in our systems and records in order to ensure adequate fulfillment of surviving provisions in terminated contracts or for other legitimate business purposes, such as to enable easier future user onboarding, demonstrate our business practices and contractual obligations, or provide you with information about our services in case of interest.

Our Digital Properties are intended for individuals 18 years of age and older. The Digital Properties are not directed at, marketed to, nor intended for, children under 18 years of age. As a general rule, we do not knowingly collect any information, including Personal Data, from children under 18 years of age. - Right to Correct. You have the right to request that we correct inaccurate Personal Data that we maintain about you.

In order to fully exercise the Right to Opt Out of Sales and Sharing for Targeted Advertising, you must undertake both of the following steps:

- Disable the use of cookies and other tracking technologies in the preference center. You must complete this step on each of our websites from each browser and on each device that you use. These steps are necessary so that we can place a first-party cookie signaling that you have opted out on each browser and each device you use. Please note:

In most cases we will collect some or all of the following data elements: first and last name, email address, and telephone number. Authorized Agents: Authorized agents may exercise rights on behalf of you by submitting a request via https://www.healthnet.com/content/healthnet/en_us/ccpa-form.html and indicating that they are submitting the request as an agent. We may require the agent to demonstrate authority to act on your behalf by providing signed permission from you. We may also require you to verify your own identity directly with us or to directly confirm with us that you provided the authorized agent permission to submit the request.

12. Disclosure of Personal Data
Although we have not "sold" Personal Data for money in the past 12 months, we engage in routine practices with our Digital Properties involving third parties that could be considered a sale of Personal Data or sharing of Personal Data for targeted advertising, as those terms are defined under applicable laws. We do not knowingly sell or share for targeted advertising any Personal Data of minors under the age of 16.
