Children’s Rehabilitative Services (CRS)

Children’s Rehabilitative Services (CRS) is a designation given to AHCCCS members who have qualifying health conditions.

Members with a CRS designation can get the same Arizona Health Care Cost Containment System (AHCCCS) covered services as non-CRS AHCCCS members and are able to get care in the community, or in clinics called multispecialty interdisciplinary clinics (MSIC).

MSICs bring many specialty providers together in one location.


Eligibility for a CRS designation is decided by the AHCCCS Division of Member and Provider Services.

AHCCCS members may receive a CRS designation when they are:

  • Under age 21; and
  • Have a CRS medical condition that qualifies

The medical condition must:

  • Need services
  • Be found by AHCCCS to meet standards as explained in R9-22-1301-1305.

Anyone can fill out a CRS application including a family member, doctor, or health plan representative.


To apply for a CRS designation mail or fax:

  • A completed CRS application; and
  • Medical documentation that supports that the applicant has a CRS qualifying condition that requires active treatment.

Call member services at 1-888-788-4408 (TTY/TDD: 711) and ask to speak with a care manager regarding CRS.

The care manager can help fill out the application, collect any medical records and submit the application for you.

Arizona Complete Health-Complete Care Plan provides medically necessary care for physical and behavioral health services and care for the CRS condition.

Members who qualify for a CRS designation and are NOT participating with DES/DDD physical health care have a choice of AHCCCS Complete Care (ACC) plans in their service area.

The ACC plan manages care for all services (including CRS, other non-CRS physical health services, and all covered behavioral health services).
